The Communities of Don Guanella and Divine Providence offer Waiver* funded Community Living Arrangements (CLA) and Intermediate Care (ICF/ID)** funded Community Homes. Individuals with intellectual disabilities benefit from full-time, experienced support staff in smaller residential settings.

At the heart of our community-based “home” is a family spirit, and a ministry delivered with love, compassion and trust in God’s Divine Providence.

Community Homes

The Community Homes are located throughout Delaware, Chester and Philadelphia Counties. An individual-centered approach to care is implemented by an interdisciplinary team that includes house managers, program specialists, nursing, psychological and behavioral support, consultants, and on-call primary care physicians. Direct Support Professionals staff the homes 24 hours per day and 7 days per week. 

Programming is guided by an individualized, person-centered support plan. Diverse opportunities for community experiences are maximized, including recreational, social, volunteer, work and worship activities.

** ICF/ID placement is Medicaid funded. Individuals must require active treatment to qualify.

Community Living Arrangements (CLA)

The Community Living Arrangement homes are located throughout Delaware County, PA. Typically, up to three people, each of whom has a private bedroom, share a home. House managers oversee the homes, and we provide an on-call primary care physician and an in-house psychiatric clinic.

An individual-centered approach to programming is implemented by an interdisciplinary team, and Direct Support Professionals staff the homes 24 hours per day and 7 days per week. Staff is selected for each home in order to create a team that best fits the individuals living there. A professional team, including nursing, psychological and behavioral support, consultant staff, and on-call primary care physicians, supports each home.

* Home and Community Based Waiver Programs allow services and supports to be provided in the community and to be paid for with Medicaid funds.
